About Us
Our Team
Grace Roots ~ Pathways to Wellness is a 501(c)3 nonprofit in Josephine County, Oregon. We are a collaborative group of individuals and organizations with vision, ideas, and compassion—taking action to meet the needs of opiate addictions in Josephine County. Our Mission: To promote the health and safety of our community by offering a continuum of services to opiate and high risk drug dependent individuals.
Our History
Grace Roots originally began as a Committee in 2014 under the former Josephine County based nonprofit agency Securing Our Safety to address the opiate addiction crisis in Josephine County. Grace Roots was incorporated as a separate nonprofit agency in 2016 and became its own 501(c)3 nonprofit in January of 2017.
Our first major project was to establish a medically assisted treatment center in Josephine County and we were successful in recruiting an existing provider from Bend to open a center in Grants Pass. The Grants Pass Treatment Center opened in a space on the north end of Grants Pass in January of 2017. Grace Roots recently opened our newest project, the Hope Home, in late summer 2025!
Our NeWEST Project!
We are very excited to announce that our vision for the Hope Home was just made a reality! Hope Home is a supportive housing program for pregnant women with opioid and other addiction challenges. We will house several women and their newborn children for up to two years while coordinating wrap around treatment and education services - addressing with a holistic approach the myriad of challenges women experience who are struggling with addiction and mothering at the same time. Hope Home just opened up to residents in the late summer of 2025!
